Program Strategies

Using a team approach to promote success, each individual has an ISP (Individual Service Plan) that outlines goals and objectives on which they work with staff daily. Quality of life is enhanced through socialization and their peers and staff, the opportunity to develop friendships and take part in recreational activities, learn new skills and participate in a variety of community activities.

Our Services

Community Choice of New Jersey provides a variety of professional supportive services in a comfortable, caring, and safe environment. Our goal is to give you a great day every day!

CCNJ community-based services include:

  • Morning and afternoon snacks
  • Catered lunch or dinner
  • Health monitoring and medication assistance
  • Goal-oriented individual care plans
  • Structured daily group and individual activities
  • Assistance with activities of daily living (ADL’s)
  • Liaison with personal physician
  • Support Groups and education for caregivers
  • Physical therapy

Additional services that may be purchased at an additional cost include:

  • Expanded Hours (more than 5 hours/day)
  • Transportation
  • Skilled Nursing Care
  • Hair Care/ Beauty Shop Services
  • Nail Care/ Foot Care
  • Special Activities (Field Trips, Ceramics, Concerts, etc.)
  • Coordination with Other Professional Services Such as Therapeutic Massage, Chiropractic Care, OT, PT, Speech Therapy, Elder Law, and Individual and Family Counseling
  • Short and Long-Term Respite/Adult Foster Care Through Help at Home Services LLC
  • Home Care Services Through Help at Home Services LLC

Eligibility

Our Day Habilitation caters to adults with one or more of the following documented disabilities with varying degrees of severity:

  • Developmental Delays
  • Intellectual Disabilities
  • Autism
  • Cognitive, Neurological, and Mental Disorders
  • Visual and/or hearing impairments
  • Traumatic Brain Injury
